Insurance Coverage and Financial Policy

Dr. Holmes is an In Network Provider for:
* Aetna
* Auto Accident PIP Claims
* Blue Cross Blue Shield
* Cigna
* First Choice Health Plans
* Premera Blue Cross
* Regence Preferred Provider Network
* Uniform Medical Plan

Insurance is a contract between you and your insurance company. Please contact your insurance company regarding your individual plan for specific benefits. Being insured is not a guarantee of coverage. Patients are encouranged to check for benefits with their insurance company.

To find out if your plan offers you coverage, look in your insurance subscriber's handbook or call the number on the back of your insurance card. Be sure to ask your insurance provider if a referral from your primary care provider is required. Ask if you have met your deductible for the year, how many visits and or treatments you will be covered for per year, and what your co pay will be for each treatment.

Insurance sees Dr. Holmes as three types of provider: Naturopathic physician, Acupuncturist, and Physical medicine provider. Please note that some insurances draw physical medicine appointments from a general pool that other providers may be billing for. These rehabilitation medicine procedures are NOT ND office visits. It is the patient's responsibility to know the type and limit of their benefits.

We will submit insurance claims for you if we are contracted with your plan or if your plan has out of network benefits; however, you are financially responsible for any balance due our office. Any balance remaining after your insurance has processed your claim will be billed to you. Your co pay will be due at the time of service.

Please be aware that your insurance plan may not cover all of the procedures Dr. Holmes may prescribe during your visit. These procedures will be your responsibility and payment will be due at the time of service.

Cancellation Policy:

24-hour Cancellation: $50
No shows: $125
*Please note that Monday cancellations need to be made 48 hours ahead of your appointment (Saturday) and Friday cancellations need to be made 48 hours ahead of your appointment (Wednesday) as the office is closed on Sundays and Thursdays. *Insurance does not cover this fee.

Payment

Herbs, supplements, and certain laboratory tests are not covered by insurance and are payable upon receipt.
